Migräne präsentiert sich als Volkskrankheit mit Kopfschmerzattacken und Auren, welche die Lebensqualität der Betroffenen deutlich beeinträchtigt. Die Frage nach der Ätiologie kann nicht schlüssig beantwortet werden. In der vorliegenden Arbeit wurden 2 Theorien der Migränepathogenese näher untersucht, um relevante Fragen für das Verständnis der Aura und des Kopfschmerzes zu klären. Die vaskuläre Theorie beruht auf der Annahme, dass eine initiale Vasokonstriktion lokale zerebrale Ischämien hervorruft, welche die neurologischen Defizite einer Aura verursacht. Die anschließende Vasodilatation wurde als Hauptursache des Kopfschmerzes konstatiert. Ziel dieser Studie war es, die Gefäßvariabilität von Migränepatienten und gesunden Personen anhand des etablierten NO-Models zu vergleichen. Hierfür unterzogen sich 28 Migränepatienten und 12 gesunde Kontrollpersonen der simultanen Untersuchung mittels TCD, NIRS und Laser-Doppler-Flowmetrie, wo sie 2 Hub Glyceroltrinitrat erhielten. Nach 4 Stunden entwickelten lediglich die Migränepatienten migränetypische Kopfschmerzen. Erstmals gelang es auch eine Aura zu induzieren. Es ließ sich kein Unterschied in der Gefäßreagibilität beider Untersuchungsgruppen nachweisen, so dass die Migräne nicht auf einer Vasolabilität bei Migränepatienten beruhen kann. Als mögliches neurophysiologische Korrelat der Migräneaura bezeichnet CSD eine neuronale Exzitationswelle, der eine längerdauernde Hemmung der kortikalen Aktivität folgt und die sich mit 2-5 mm/min unabhängig von vaskulären Territorien über den Cortex ausbreitet. Bei gleicher Ausbreitungsgeschwindigkeit der Skotome einer visuellen Aura wurde ein Zusammenhang zwischen beiden Phänomenen postuliert.
