This item discusses and demonstrates with diagrams and a prosected cadaver, the anatomy and surgical approach to both the sacroiliac joint and the spine. The anatomical features of the muscles, ligaments, and nerves encountered as well as the structures used as anatomical landmarks are discussed. In the first procedure, the incision begins over the lumbosacral region above the line joining the posterior-superior iliac spines and extends downward to the greater trochanter, access is thus obtained to the sacroiliac joint. Emphasis is placed on identifying the anatomical structures encountered in this approach, particularly the gluteus medius and gluteus maximus. Simulated bones are used to illustrate the articulation of the sacrum and the ilium. The second procedure provides access to the rectal peritoneal area. In this approach, the incision begins just above the anterior-superior spine of the ilium and extends obliquely midway between the iliac crest and the twelfth rib to the costal vertebrae. This approach is also useful in exposing other structures such as the kidney and ureter
