Paul TESSIER died on 2008 January and left us a impressive surgical heritage considering surgical procedure, surgical think and inspiration, an eternal leadership but also an historic collection. The TESSIER collection in now available in Amiens since it is the property of the French Association of Maxillo-Facial Surgeons, founded by Paul Tessier in particular . This collection constitutes obviously a reference for many studies, analysis and works in all aspects of the speciality. It is constituted of 20 m3 documents, drawings, medical files, surgical tool prototypes, malformation surgery and plastic surgery iconographies, historic pictures library of skulls coming from all around the world and a exceptional skull collection. We take here the opportunity to illustrate the evolution of Paul Tessier thinking regarding the conception and then the management of surgical treatment for major craniofacial malformations through the analysis of those archives

Le remplacement a longtemps été considéré uniquement comme un passage obligatoire avant une installation libérale. Dans le contexte actuel de mesures gouvernementales visant à favoriser l'installation ou le regroupement dans les zones sous-médicalisées, nous avons réalisé une enquête auprès des remplaçants en médecine générale et des omnipraticiens récemment installés en Picardie. Celle-ci met en évidence un retard croissant à l'installation ainsi qu'une attractivité relative de l'exercice libéral face aux attraits d'un mode d'exercice salarié. Il ressort que les facteurs déterminants à cette installation chez les jeunes praticiens qui redoutent l'isolement d'un exercice solitaire sont au moins autant d'ordre personnel que professionnel. Il a été mis en évidence que si les mesures proposées (dont certaines ne sont pas fixées de façon définitive à ce jour) ne semblent pas suffisantes pour induire une modification marquée des comportements d'installation, il semble néanmoins possible de réduire les inégalités. Nous ne pouvons que souhaiter l'efficacité de telles mesures ce qui permettrait d'éviter une remise en cause de la liberté d'installation qui nous semble constituer un des fondements de la médecine libérale actuelle

Documentary looking at the realm of facial transplantation surgery. In Amiens, France, November 2005, Isabelle Dinoire had a partial face transplantation. Professor Guo Shuzhong China's leading plastic surgeon at Xijing Hospital, Xi'an, has a ward full of patients with severe facial disfigurements. He is very keen to perform a full facial transplantation and takes on the case of Li Guoxing who was terribly disfigured after being attacked by a bear in 2004. He needs a full face transplantation including the bones of the nose - the 18-hour surgery is shown in some detail. Li returns to his village home but it is clear that the remote location will be difficult for him to gain access to the kind of medication he needs. Two years after his surgery his face begins to reject the transplanted tissue as he has not been taking the medication necessary to suppress his immune response to the new tissue. Reportedly, Li died last year. The documentary also brings us up to date with Isabelle Dinoire, 2.5 years after her surgery as she receives facial physiotherapy

Enophtalmos' management is a real challenge of clinical and radiological analysis. A first study is composed of 86 patients who have undergone ocular socket's reconstruction with orbital implant and prosthesis, at the university hospital of Amiens. It gives the opportunity to create a classification of different morphological type of orbit and a clinical score of enophtalmos' degree, that become precious devices for photographic analysis. In a geometrical approach, the morphological type explains the occurrence and the severity of enophtalmos. The volume loss is visible on the entire orbitopalpebral surface of the socket that is more or less exposed depending on this type : wider on an "opened" and large orbital shape but narrowed around the anteroposterior axis on a "closed" shape. After primary surgery followed by achievement of prothesis rehabilitation, the surgeon's role is to explore the different surgical procedures available involving the bone container or the soft tissue content to provide prosthesis' stability and to treat the residual enophtalmos. That's the subject of a second study composed of 34 patients operated by Dr Sorrel-Déjerine in Paris. The detailed analysis lead to the development of a surgical strategy that is synthesized on a algorithm. The correction procedures are exposed and illustrated by examples that highlight the difficulty to assess with objectivity the missing volume to return and it's topography. So, we take advantage of the imagery's techniques (CT scan, MRI...) to evaluate this volume in the orbital space. For this purpose, a protocol has been conceived to locate the volume loss in the different orbital compartments. But for an evaluation of the oculopalpebral surface, the photogrammetry is a precious realistic device to situate the visible defect in 3D by comparison between the two sides. The next step involves the analysis model of orbital biomechanics that simulate the volumes moving in the orbital cavity for a better anderstanding of the anophtalmic socket's physiopathology. Those devices are exposed here to help the specialist concerned in enophtalmos' treatment of anophtalmic patients, to do an objectiv analysis with a surgical planification at each step of correction. The purpose is to provide the best choice of procedure in primary care and secondary volume enhancement surgery

The temporomandibular joint (TMJ), histologically similar to other joints of the human body is a complex joint due to its embryological origin, due to its anatomy and biomechanics of the hand making it ultimately unique. Thus, conditions affecting the joints are still poorly understood, often misdiagnosed and poorly defined above. Ankylosis of the temporomandibular joint is no exception. Reached rare, the term remains ambiguous, misused, its definition is still under debate today. The treatment of this disease, for his undoubted remains surgical. However, the complexity of this joint seems to be a brake on reconstructive surgical techniques. Yet from the beginning nineteenth century, this surgery has seen many of the techniques described: the simple act of interposition and condyloplastie (interposition of autologous or alloplastic material) to complex gestures (bone grafts, free flaps), and the use of joint prostheses. The number of joint replacements and the number of surgical techniques described shows the perfectibility of each technique, requiring constant development of new approaches reconstructive. This work concerns the analysis of 16 patients treated between 1998 and 2010. Using tools developed at the Technological University of Compiègne to analyze the articular surfaces of the ATM, we will show that new technologies, including 3D models using geometric and finite element allows support for both the diagnostic and therapeutic, but also the development of new joint prostheses in reconstructive surgery of the TMJ. Algorithm and a decision will be exposed to different techniques of reconstruction after resection of the TMJ ankylosis block depending on the type and etiology with a companion case by age and surgical history. Based on new technologies and customized prosthetic research in the field of tissue engineering, the goal should be that of the manufacture of a biological substitute for meeting the requirements osteochondral biomechanics of the TMJ
Fentes orbito-faciales rares évaluation de la prise en charge à long terme : à propos de 15 observations by Caroline Racz( Book )

Les fentes faciales sont des affections rares et décrites à travers peu de cas rapportés dans le monde. Même si plusieurs hypothèses ont été avancées, leur étiopathogénie reste inconnue à ce jour. La variété des présentations cliniques et des terminologies pour les décrire n'ont jamais simplifié l'accès à une classification reconnue et utilisée par tous. Tessier, en 1976, publie une classification basée sur des observations cliniques et anatomiques, et différencie alors quinze localisations de fentes faciales rares, qui sont nommées de 0 à 14 autour de l'orbite. Nous avons choisi d'étudier 15 cas de fentes orbito-faciales (fentes 3, 4 ou 5) pour la complexité de leur prise en charge, et de les évaluer sur le long terme. L'étude de nos observations et de la littérature a montré qu'aucun cas de fente orbito-faciale ne possède les mêmes caractéristiques cliniques. En pratique, la grande variété des présentations cliniques ne permet pas d'aboutir à un algorithme de prise en charge chirurgicale selon le type de fente, mais plutôt selon le type d'atteinte, variable au sein d'une même fente. Nous avons tenté d'illustrer la complexité de la prise en charge globale de ces patients en confrontant l'analyse de leurs caractéristiques à celles décrites initialement par Tessier pour sa classification. Nous avons également analysé la difficulté de la reconstruction de ces fentes faciales dont le résultat est sans cesse soumis à la croissance pathologique des tissus. Indépendamment de la nécessité d'une prise en charge globale à long terme, nous avons enfin cherché à discuter de l'utilité des classifications pour l'étude clinique de ces patients
