Im Mittelpunkt des Geschehens steht der Arzt Bernard Rieux: Schon zu Beginn, als noch niemand in der Stadt auch nur an die Pest denkt, steht seine Diagnose fest: Den dahingerafften Ratten werden die Menschen folgen! Doch bis die Behörden endlich in die Gänge kommen, vergeht kostbare Zeit. Unermüdlich versucht Rieux zu retten. Um den Arzt gruppieren sich der zufällig in der Stadt weilende Journalist Rambert, der anfangs zu seiner Geliebten nach Paris fliehen will, dann aber bleibt, der Jesuitenpater Paneloux und Tarrou. Auch sie setzen alles daran, Menschen zu retten. Dramatischer Höhepunkt der tödlichen Entwicklung ist das sinnlose Sterben eines kleinen Jungen. Sinnlos und absurd ist auch das Sterben des Paters, des Freundes Tarrou und der Ehefrau des Arztes. Kurz vor Ausbruch der Pest war sie in ein Sanatorium außerhalb der Stadt gegangen. Am Ende gibt sich der Verfasser der Chronik als der Arzt Rieux zu erkennen, für den die Pest eine endlose Niederlage war und ist. Denn "der Pestbazillus stirbt niemals aus oder verschwindet ..."
