La sepsis bacteriana neonatal constituye una de las principales causas de muerte, o de secuelas muchos veces irreversibles, despues de las primeras 72 horas de vida. Debido a que el diagnostico precoz es crucial para mejorar el pronostico de los neonatos infectados, y que los metodos actuales no presentan la necesaria sensibilidad y rapidez, el objetivo del presente trabajo fue analizar la posible utilidad de la determinacion de Interleuquina 6 (IL6) y del Receptor soluble de Interleuquina 2 (sIL-2R) como marcadores diagnosticos y pronosticos de sepsis neonatal y, asimismo, efectuar la comparacion con un marcador clasico de infeccion bacteriana como es la Proteina C Reactiva (PCR). Se estudiaron cuatro grupos de recien nacidos, comprendidos en la siguiente clasificacion clinico-epidemiologica: sepsis confirmada (SC), n=22, sepsis muy probable (SMP), n=18, sepsis probable (SP), n=11 y sin infeccion bacteriana (SIB), n=13. Mediante inmunoensayo enzimatico se realizo el dosaje de los niveles sericos de IL6 y sIL-2R y por inmunodifusion la determinacion semicuantitativa de PCR. Se calculo el limite de corte para cada determinacion con los valores obtenidos en los neonatos no infectados y para el calculo de sensibilidad, especificidad y valor predictivo se tomo como referencia el resultado del analisis microbiologico. Los siguientes resultados fueron obtenidos en las muestras tomadas en el momento de la admision: sensibilidad: IL6 90 por ciento, sIL-2R 60 por ciento, PCR 41 por ciento; especificidad: IL6 50 por ciento, sIL-2R 100 por ciento, PCR 100 por ciento; valor predictivo negativo: IL6 75 por ciento, sIL-2R 58 por ciento, PCR 59 por ciento. Para evaluar el comportamiento como marcadores evolutivos, se tomo una muestra a las 48 horas, cuando los pacientes de los grupos SC, SMP y SP estaban bajo tratamiento antibiotico. Los resultados mostraron que IL6 descendio en los neonatos con evolucion favorable e incremento marcadamente en los que tuvieron desenlace fatal; sIL-2R mostro un comportamiento inverso y PCR no sufrio variaciones detectables en el periodo estudiado. Los resultados obtenidos en el presente trabajo sugieren que IL6 puede ser un buen marcador de sepsis neonatal, fundamentalmente por su elevada sensibilidad, y que la especificidad puede incrementarse mediante el estudio de un perfil de marcadores. Abstract : Supplemental Digital Content is available in the text Abstract : Abstract: Minimally invasive surgery (MIS) of colorectal cancer (CRC) was first introduced over 20 years ago and recently has gained increasing acceptance and usage beyond clinical trials. However, data on dissemination of the method across countries and on long-term outcomes are still sparse. In the context of a European collaborative study, a total of 112, 023 CRC cases from 3 population-based (N = 109, 695) and 4 institute-based clinical cancer registries (N = 2328) were studied and compared on the utilization of MIS versus open surgery. Cox regression models were applied to study associations between surgery type and survival of patients from the population-based registries. The study considered adjustment for potential confounders. The percentage of CRC patients undergoing MIS differed substantially between centers and generally increased over time. MIS was significantly less often used in stage II to IV colon cancer compared with stage I in most centers. MIS tended to be less often used in older (70+) than in younger colon cancer patients. MIS tended to be more often used in women than in men with rectal cancer. MIS was associated with significantly reduced mortality among colon cancer patients in the Netherlands (hazard ratio [HR] 0.66, 95% confidence interval [CI] (0.63-0.69), Sweden (HR 0.68, 95% CI 0.60-0.76), and Norway (HR 0.73, 95% CI 0.67-0.79). Likewise, MIS was associated with reduced mortality of rectal cancer patients in the Netherlands (HR 0.74, 95% CI 0.68-0.80) and Sweden (HR 0.77, 95% CI 0.66-0.90). Utilization of MIS in CRC resection is increasing, but large variation between European countries and clinical centers prevails. Our results support association of MIS with substantially enhanced survival among colon cancer patients. Further studies controlling for selection bias and residual confounding are needed to establish role of MIS in survival of patients

Se realizo un estudio retrospectivo de 121 pacientes con meningitis bacteriana neonatal(MBN)diagnoticadas en un periodo de 12 anos, a fin de establecer bases etiologicas para su tratamiento. Fueron clasificados segun ecosistemas de origen en:(n=29,23,9 por ciento)hospitalarios(n=26,21,4 por ciento)y comunitarios(n=17,14 por ciento). Otros 49(40,4 por ciento)con aislamiento bacteriano en LCR obtenido postmorten, fueron clasificados como infecciones terminales. Las especies prevalentes en MB perinatal fueron streptococcus agalactie, escherichia coli, enterococcus faecalis y listeria monocytogenes. En Mb de origen nosocomial las especies prevalentes fueron staphylococcus aureus, staphylococcus epidermis, enterobacter spp y pseudomonas spp. En meningitis de origen comunitario, las especies prevalentes fueron streptococcus spp. En nuestra unidad, el esquema de ampilicilina y cefalosporina de tercera generacion fue adecuado como terapeutica inicial en neonatos con meningitis bacteriana de origen perinatal y comunitario. En la de origen nosocomial las drogas deben indicarse, de acuerdo a los resultados bacteriologicos, para cada caso individual
