Chronic heart failure, a syndrome with increasing incidence and prevalence, represents the main cause of cardiovascular morbidity and mortality. It is a complication of spectrum of etiologically various diseases which, in syndrome progression, include complex pathophysiological mechanisms. Newer data indicate that aldosterone plays important and partly from kidney independent role in heart failure progression. Binding to mineralocorticoide receptors aldosterone causes direct harmful effects to myocardium and vasculature. Aldosterone is coresponsible for cardiac hypertrophy, necrosis, fibrosis, cardiac remodeling and endothelial dysfunction, factors known to contribute tothe pathophysiology of development and progression of heart failure. Clinical and detailed mechanistic studies performed by the end of millennium, have proved mineralocorticoid receptor antagonism to be a key component of CH F treatment regimen. It seems reasonable to expect aldosterone receptor antagonists to become a part of a new therapeutic paradigm along the whole range of cardiovascular diseases, together with already established ACE inhibitors, angiotensin II receptor blockers and beta blockers

Tissue Doppler echocardiography is a modification of the conventional colour Doppler technology, which is used for the measurement of cardiac tissue velocities. Tissue velocities are considerably lower, but with the amplitudes which are much higher than those recorded from blood flow. Tissue Doppler echocardiography has the potential to assess the contractions and relaxations of the left and right ventricles, the rate of myocardial thickening, and transmural velocity gradients, and can also be used to measure systolic and diastolic time intervals on a regional basis. In the management of patients with chronic aortic regurgitation and when selecting of the most appropriate time for surgical procedures, several facts should be considered. The atricle emphasises a comprehensive evaluation of clinical and echo-Doppler parameters. Aortic regurgitation leads to a progressive decline in left ventricle pump function secondary to chronnic volume overload and excessive afterload, as well as left ventricle contractiledysfunction if the disease is prolonged. Sometimes, subclinical left ventricle conctactile dysfunction may develop when the left ventricle injection fraction remains normal, but it is generally improved after valve replacement. Asymptomatic patients with normal left ventricle ejection fraction have an excellent long-term prognosis with vasodilator therapy for afterload reduction. Valve replacement in symptomatic patients with mild to moderately depressed left ventricle pump function improves survival over medically treated patients with aortic regurgitation. Severe preoperative contractile dysfunction is associated with persistent left ventricle dilatation and dysfunction, often resulting in postoperative congestive heart failure and death

The modern treatment of chronic heart failure is directed towards both high morbidity and mortality of this syndrom. Only therapy with proper dosages of ACE inhibitors, beta adrenergic blockers and spironolacton reduce morbidity aswell as mortality of this condition. Gibljivi trombi v srcu so odlitki perifernih in medeničnih ven, ki se na poti v pljuča predhodno ustavijo v srcu. Čeprav jih redko vidimo, so klinično pomembni, ker napovedujejo masivno pljučno embolijo in visoko umrljivost. Kronično srčno popuščanje (KSP) je glavni vzrok obolevnosti in umrljivosti v razvitih deželah. KSP je sindrom, ki poteka maligno. Povprečna letna umrljivost bolnikov s tem sindromom je 10%. Kakovost življenja je bolj okrnjena kot pri kateri koli drugi bolezni. KSP je najpogostejši vzrok hospitalizacije pri bolnikih, starejših od 65 let V zadnjih desetletjih se je število hospitalizacij zaradi KSP trikrat povečalo. Hospitalizacije zaradi KSPso pogoste, dolge in se ponavljajo. Vzroki za hospitalizacijo bolnikov s KSP so diagnostični postopki v zvezi z osnovno boleznijo, uvajanje zdravil priogroženih bolnikih in neobvladljivo srčno popuščanje. Osnovno bolezen pogosto potrdimo, natančno opredelimo, včasih pa šele odkrijemo z ehokardiografijo, ki je izvedljiva ambulantno. Zelo pomembno je, da lahko s toenostavno preiskavo opredelimo delež sistolične in diastolične disfunkcije srčne mišice pri srčnem popuščanju. Najpogostejša diagnostična preiskava, zaradi katere bolnike hospitaliziramo, je koronarografija. Potrebna je pri bolnikih, kjer sumimo na koronarno bolezen. Večini bolnikov s KSP lahko uvedemo zdravljenje ambulantno, le ogroženim bolnikom, predvsem tistim z okrnjeno sistolično funkcijo, uvajamo zaviralce ACE in blokatorje beta v bolnišnici. Začetni odmerki teh zdravil morajo biti majhni, ciljni odmerki, kijih moramo doseči, pa so veliki. Neobvladljivo srčno popuščanje je najpogostejši vzrok hospitalizacije. Vedno moramo odkriti in zdraviti oz. odpraviti sprožilne dejavnike, med katerimi so najpogostejši čezmerno uživanjesoli in vode, ishemija miokarda, neurejena arterijska hipertenzija in aritmije. V bolnišnici zdravimo hudo prizadete bolnike s sekvenčno diuretično terapijo, večamo odmerke zaviralcev ACE, zdravimo aritmije, po potrebi posežemo po inotropnih in vazodilatacijskih zdravilih. V skrajnem primeru, če so pogoji, se odločimo za transplantacijo srca

In the management of patients with chronic mitral regurgitation and selection of the most appropriate time for surgical interention in these patients, several facts discussed in the article should be considered. Evaluation on thebasis of clinical and echo-Doppler parameters is emphasised. Mitral regurgitation leads to an eccentric hypertrophy and progressive decline in left ventricle pump function secondary to contractile dysfunction. Patients with a normal left ventricle pump function and a reversible contractile dysfunction have a favourable long-term postoperative death becouse of the persistent left ventricle dysfunction, but the survival rate is improved with surgical therapy in comparison with medical therapy. Mitral valve reconstruction preserves the left ventricle function and reduces mortality andcomorbid complications in comparison with conventional mitral valve replacement. Mitral valve reconstruction shoul be the procedure of choice in patients with reduced preoperative left ventricle function
