Chronic heart failure, a syndrome with increasing incidence and prevalence, represents the main cause of cardiovascular morbidity and mortality. It is a complication of spectrum of etiologically various diseases which, in syndrome progression, include complex pathophysiological mechanisms. Newer data indicate that aldosterone plays important and partly from kidney independent role in heart failure progression. Binding to mineralocorticoide receptors aldosterone causes direct harmful effects to myocardium and vasculature. Aldosterone is coresponsible for cardiac hypertrophy, necrosis, fibrosis, cardiac remodeling and endothelial dysfunction, factors known to contribute tothe pathophysiology of development and progression of heart failure. Clinical and detailed mechanistic studies performed by the end of millennium, have proved mineralocorticoid receptor antagonism to be a key component of CH F treatment regimen. It seems reasonable to expect aldosterone receptor antagonists to become a part of a new therapeutic paradigm along the whole range of cardiovascular diseases, together with already established ACE inhibitors, angiotensin II receptor blockers and beta blockers

In the management of patients with chronic aortic regurgitation and when selecting of the most appropriate time for surgical procedures, several facts should be considered. The atricle emphasises a comprehensive evaluation of clinical and echo-Doppler parameters. Aortic regurgitation leads to a progressive decline in left ventricle pump function secondary to chronnic volume overload and excessive afterload, as well as left ventricle contractiledysfunction if the disease is prolonged. Sometimes, subclinical left ventricle conctactile dysfunction may develop when the left ventricle injection fraction remains normal, but it is generally improved after valve replacement. Asymptomatic patients with normal left ventricle ejection fraction have an excellent long-term prognosis with vasodilator therapy for afterload reduction. Valve replacement in symptomatic patients with mild to moderately depressed left ventricle pump function improves survival over medically treated patients with aortic regurgitation. Severe preoperative contractile dysfunction is associated with persistent left ventricle dilatation and dysfunction, often resulting in postoperative congestive heart failure and death

Mitral valve reconstruction shoul be the procedure of choice in patients with reduced preoperative left ventricle function

Floating cardiac thrombi are peripheral and pelvic venous clots which, on their way to the lungs, are accidentally lodged in the heart. They are rare but clinically significant because of the high incidence of massive pulmonary embolism and high mortality. Two patients, one with a snake-like thrombus in the right atrium and one with a snake-like thrombus in the left atrium stuck in the open foramen ovale, are presented and discussed. The significance of early echocardiographic examination in suspicion of a pulmonary embolism is stressed. If there are floating emboli in the heart, emergency care is required, but there is no clear consensus regarding the optimal theapeutic approach
