Abstract:

After decades of ongoing research in the area of child maltreatment and in traumatology, the mental health community continues to struggle diagnostically with how to conceptualize the often pervasive and co-morbid sj'mptom pattern associated with child abuse and neglect (Herman, 1992a, 1992b; 1997; Messler-Davies & Frawley, 1994; Van der Kolk, McFarlane, & Weisaeth 1996). In fact, there is no current category for their constellation of symptoms. Instead, the Diagnostic and Statistical Manual-Fourth Edition-Text Revision (DSM-IV-TR) (APA, 2000) briefly describes this symptom pattern in a paragraph entitled, "Associated features ... ofPTSD" (APA, 2000). It has a unique diagnostic presentation that mimics symptoms of both Borderline Personality Disorder (BPD) and Bipolar Disorder (BD) (Van der Kolk et al., 1996). The purpose of this study was to compare clinicians' mean diagnostic certainty across two trauma conditions: simple and complex trauma. The sample consisted of 63 mental health workers with five years post Master's licensure. Major hypotheses included: (1) Mean diagnostic certainty for PTSD will be significantly greater in the simple trauma condition (2) Mean diagnostic certainty for BPD and BD will be significantly greater in the complex trauma condition. Results indicate that while mean diagnostic certainty for PTSD remained the same across symptom conditions, mean diagnostic certainty for BPD and BD increased significantly in the complex condition, suggesting that in the absence of sufficient diagnostic categories, clinicians utilize multiple diagnostic categories to account for a complex trauma symptom pattern.
