This is an assessment that looks at the level of support a person needs once they return home by looking at safety, self-care, money management, transportation and telephone use, and work and leisure. On the 16 questions, a person is scored either independent, or needs assistance.
I did a paper on this assessment and feel that it can be a useful screening tool for occupational therapist who are discharging patients however, it is quite out-dated at this point and was never well researched.
