Comparison of platelet intracellular calcium changes among groups of depressed and hypertensive veterans by Karen Russell( Archival Material )

Essential hypertension is a prevalent, multiply determined, medical condition that is a known risk factor for cardiovascular, cerebrovascular and renal impairment. Evidence is reviewed that suggests a link between depression and subsequent hypertension development. Findings of similar forms of platelet intracellular calcium dysregulation in both disorders suggests a potential biological basis for this connection. To date, no study has controlled for depression prior to assessing platelet intracellular calcium levels among hypertensive patients. Furthermore, platelet samples from depressed and hypertensive patients have yet to be directly compared. This study addressed these concerns by comparing serotonin-stimulated platelet intracellular calcium changes among patient groups suffering from depression (n = 18), hypertension (n = 18), both disorders (n = 18), or neither disorder (n = 16) . Resting and serotonin-stimulated platelet intracellular calcium levels were computed for each subject using Fura-2 loaded platelets. Between group comparisons were then made. Correlations between platelet intracellular calcium change, depression and hypertension status were examined. Platelet intracellular calcium increases within both depression groups were significantly greater than platelet intracellular calcium increases exhibited by controls and by patients in the hypertension alone group. Patients with hypertension alone did not demonstrate greater average platelet intracellular calcium increase than control subjects free of hypertension and depression. Depression level was significantly related to platelet intracellular calcium increase across groups, r(70) = .57, p <.001. However, hypertension status, systolic blood pressure level and diastolic blood pressure level did not correlate significantly with platelet intracellular calcium change. Study results suggest that the association between heightened platelet intracellular calcium responding and hypertension is a spurious one that is eliminated once the influence of depression is removed. Alternate interpretations of study findings are also discussed. Current findings are consistent with the notion that depression may function as an independent risk factor for cardiovascular impairment and that this association may be mediated, in part, by platelet intracellular calcium regulation abnormalities
