Interpersonal problems as they affect the development of therapeutic alliance and group climate in group psychotherapy for women survivors of childhood sexual abuse by Ruth Nevo( Book )

Childhood sexual abuse (CSA) adversely affects psychological well being and specifically leads to interpersonal difficulties, which in turn are likely to affect survivor's experience of the therapeutic process. Recognizing that the therapeutic process is a dynamic one, this study investigated the nature of the relationship between participants pre-therapy interpersonal problems and the temporal development of both the therapeutic alliance and group climate in a population of adult survivors of CSA. Forty-eight treatment seeking women who reported a history of CSA participated in a course of six months of group therapy for women survivors of CSA. Participants completed the Inventory of Interpersonal Problems (Horowitz, Rosenberg, Baer, Ureno, & Villasenor, 1988) prior to beginning group therapy and repeated administrations of the Group Climate questionnaire - Short Form (MacKenzie, 1983) and the California Psychotherapy Alliance Scale - Short Form (Gaston & Marmar, 1987) throughout the six months of therapy. Hierarchical linear modeling identified growth patterns of alliance and group climate and related these patterns to participant's pre-therapy interpersonal problems. Results provide further evidence to support the temporally dynamic nature of both alliance and group climate and promote our understanding of the role interpersonal problems play in shaping participants' experience of the therapeutic process. Specifically it was found that interpersonal problems in the dominance end of the control dimension predict higher initial alliance ratings, higher GCQ-Engaged ratings and higher GCQ-Conflict ratings at the onset of therapy. Such problems also tended to predict rising alliance levels over the course of therapy and a reduction in GCQ-Conflict ratings over time. Interpersonal problems in the friendly end of the affiliation dimension predicted higher initial alliance ratings at the onset of therapy as well as higher GCQ-Engaged ratings at the onset of therapy. Such problems also predicted decreased alliance and GCQ-Conflict ratings over time, and increases in GCQ-Engaged ratings over time. Findings at the octant level of the IT are also significant and resemble findings on the dimensional level. Results are discussed in relation to participants' trauma history, its effects on interpersonal functioning and, in turn, on the psychotherapy experience
