En mayo de 2002 fue presentada en la reunion anual de la American Society of Clinical Oncology la nueva clasificacion TNM de melanoma. Utilizar la nueva clasificacion en pacientes con melanoma para evaluar su aplicabilidad y su valor pronostico. En este estudio se recopilaron los datos de las historias clinicas de 35 pacientes que ingresaron al Instituto de Oncologia Luis Razetti entre 1997 y 2001 con diagnostico de melanom para adaptacion a la nueva clasificacion. Se evaluaron: sexo, edad, lugar de diagnostico, tipo de biopsia, tiempo de evolucion, ubicacion superficie, satelitosis, metastasis ganglionares o a distancia, profundidad de invasion, ulceracion, subtipo histologico, modalidad de tratamiento, para compararalos en cuanto a sobrevida global y libre de enfermedad. La superficie tumoral (P < 0,01), satelitos (P < 0,05), ganglios positivos (P < 0,05), metastasis a distancia (P < 0,01) y profundidad de invasion (P< 0,01) fueron factores que influyen significativamente en la evolucion de la enfermedad. El sexo masculino (P = 0,054) y las metastasis en transito (P = 0,057), fueron factores de mal pronostico. El tipo de biopsia, tiempo de evolucion, ubicacion de la lesion, subtipo histologico, presencia de ulceracion, modalidad de tratamiento, no fueron factores determinantes en el comportamiento de la lesion. Nuestros resultados, en su mayoria, son comparables con los de la literatura internacional, a excepcion de la superficie de la lesion primaria, que fue factor determinante en el comportamiento de la enfermedad(AU).

Evaluar los factores pronosticos de los sarcomas de partes blandas en extremidades, en el Instituto de Oncologia "Dr. Luis Razetti". Estudio retrospectivo de 120 pacientes con diagnostico de sarcoma de partes blandas en extremidades, tratados en el Instituto de Oncologia "Dr. Luis Razetti" entre enero de 1979 y diciembre de 1995. Se revisaron variables clinicas y patologicas para identificar los factores predictivos de sobrevida global, sobrevida libre de enfermedad, recaida metastasica y recaida local. De los pacientes 56 por ciento del sexo masculino y 44 por ciento del sexo femenino: media de edad 36 anos; sitio anatomico mas frecuente afectado: el muslo (43 por ciento). Con una media de seguimiento de 43 meses, la recaida local fue del 24 por ciento, recaida metastasica del 41 por ciento. Fallecieron 48 pacientes (40 por ciento). La sobrevida global y sobrevida libre de enfermedad actuarial a 5 anos fue de 60 por ciento y 49 por ciento, respectivamente. Los factores relacionados con recaida local fueron los margenes quirurgicos positivos y los sub-tipos histologicos schwanoma maligno y sarcoma sinovial. Para recaida metastasica y sobrevida global fueron alto grado tumoral, tamano tumoral > 5 cm, penetracion tumoral, y las variedades histologicas leiomiosarcoma y schwanoma maligno. Fueron factores adversos de sobrevida libre de enfermedad alto grado histologico, margenes, quirurgicos positivos, tamano tumoral > 5 cm, penetracion tumoral y las variedades histologicas leiomiosarcoma y schwanoma maligno. El alto grado histologico es el principal factor predictivo para recaida metastasica, sobrevida global, sobrevida libre de enfermedad, mientras que el margen quirurgico lo es para recaida local(AU).
