This is the second edition of the original textbook on "evidence-based medicine". An investigation into the working of the clinical sector of the NHS strongly suggests that the simplest explanation of the findings is that this sector is subject to a severe inflation with the output rising much less than would be expected from the input. The evidence is based on a historical survey, crude input and output measurements, and measurements of effectiveness and efficiency in the diagnosis and treatment of some common diseases. This does not amount to proof, but is reasonable conclusive. It is suggested that the inflation could be controlled by science, in particular by the wide use of randomized controlled trials. This new edition of Cochrane's work contains a new introduction by Professor Chris Silagy and a foreword by Dr. Ian Chalmers
