This book, an extended collection of lectures delivered at "Schloss Reisensburg" during the DMV-Seminar "Algebraic Geometry, 1991", aims at presenting Kodaira's vanishing theorem and several generalizations in a way which is as algebraic as possible. We develop the theory of logarithmic de Rham complexes, the use of the corresponding spectral sequence, lifting properties for manifolds and their Frobenius morphisms in characteristic p>0 and the proof of the degeneration of the Hodge to de Rham spectral sequence with algebraic methods, due to P. Deligne and L. Illusie. We apply those methods to obtain vanishing theorems. Several typical applications and the generic theorems of M. Green and R. Lazarsfeld complete the picture. This exposition is self-contained and accessible to anyone with background in modern algebraic geometry. The necessary formalisms from cohomology theory are recalled in an appendix. Contexte : L'infection à Chlamydia trachomatis (Ct) est l'infection sexuellement transmissible (IST) la plus répandue chez les moins de 25 ans. Du fait de ses morbidités et de son expression peu symptomatique, l'IST à Ct est un problème de santé publique. En France, il n'existe pas de recommandation pour un dépistage systématique en population générale. D'ailleurs un tel dépistage n'apparait pas raisonnable en termes de coûts. C'est dans ce contexte qu'a été développé un outil d'interrogatoire standardisé Il nous semblait intéressant de tester cet outil en médecine préventive universitaire (MPU), lieu fréquenté par une population jeune donc à priori à risque. Objectifs : Estimer la prévalence de l'infection en MPU. Etudier les facteurs de risques que sont l'âge et la durée écoulée depuis le dernier changement de partenaire. Etudier la performance de l'outil d'interrogatoire standardisé. Observer ce que peut apporter l'introduction de ce type d'outil sur la pratique de ce dépistage. Méthode : Nous avons réalisé une étude descriptive analytique, observationnelle et transversale, au sein du service de MPU de la faculté de Poitiers, du 1er octobre 2012 au 28 juin 2013. Nous avons recueilli 2 facteurs de risque que sont l'âge et la durée de la dernière relation, le taux de positivité et la présence ou non des principaux symptômes de l'infection. Résultats : Deux cent quatre-vingt-seize femmes et 117 hommes ont été inclus. La moyenne d'âge était de 21.8 ans pour les femmes et 22.9 ans pour les hommes. La moyenne de la durée depuis le dernier changement de partenaire était de 6.6 mois pour les femmes et 4 mois pour les hommes. La prévalence de l'infection était de 7.5%. Il n'y avait pas de différence statistiquement significative entre le groupe positif et le groupe négatif concernant la présence de symptômes en association avec l'infection. La petite taille de notre échantillon ne nous a pas permis d'effectuer tous les calculs nécessaires à l'étude de la performance de notre outil. Conclusion : Nous avons observé un biais de sélection : notre population était majoritairement une population ayant consulté au sein du CDAG de ce MPU et elle en avait les caractéristiques. Dans ce contexte, les critères de l'âge et de la durée depuis le dernier changement de partenaire étaient pertinents puisque 96% des patientes étaient dépistées. Cependant, en termes de coûts, ces critères posent problème puisqu'ils conduiraient à proposer une PCR à 70% de la population consultant en MPU. Notre outil pourrait trouver son utilité dans une démarche de rationalisation des coûts de soins. Il pourrait également trouver une utilité pour son effet « reminder ». Son intérêt comme outil d'amorce à l'abord de la sexualité doit être testé. D'autres études sont nécessaires pour évaluer la performance de l'outil en soins primaires

This Ph. D. thesis studies the arithmetic properties (the Hasse principle, the weak approximation, and the Brauer-Manin obstruction) for zero-cycles on algebraic varieties defined over number fields. We introduce the notion of generalized Hilbertian subset. By using the fibration method, we prove that the Brauer-Manin obstruction is the only obstruction tothe Hasse principle and to the weak approximation for zero-cycles of degree 1; and establish the exactness of a sequence of global-local type concerning Chow groups of zero-cycles, for certain varieties which admit a fibration structure overa smooth curve or over the projective space, where the arithmetic hypotheses are only posed on the fibers over a generalized Hilbertian subset. Moreover, we relate the arithmetic of rational points and that of zero-cycles of degree 1 on geometrically rationally connected varieties. As an application, we find that the Brauer-Manin obstruction is the only obstruction to the Hasse principle and to the weak approximation for zero-cycles of degree 1 on- homogeneous spaces of a linear algebraic group with connected stabilizer,- certain varieties fibered into Chatelet surfaces over a smooth curve or over the projective space (in particular, Poonen's threefolds)
