Identificacion de enteropatogenos en la diarrea de la infancia en un estudio realizado en la ciudad de Posadas, Misiones by Marta Vergara( )

El presente trabajo informa de los resultados de aislamientos de Enteropatogenos en ninos menores de cinco anos, sin tratamiento antibiotico previo, con menos de siete dias de evolucion de su diarrea, internados o ambulatorios del Hospital R. Madariaga, desde junio de 1986 hasta enero de 1987. Estudio que continua hasta la fecha. Del total de 252 casos estudiados, resultaron 65,5%de aislamientos positivos para Esteropatogenos aislados. De la evaluacion realizada surge que Shigella flexneri serotipo 2 ocupa el primer lugar con respecto a Shigella, siendo el serotipo 2 el que produce cuadros mas severos de enteritis, datos que concuerdan con el resto del pais. Del analisis de Escherichia coli EPI, se destaca la mayor frecuencia del serotipo 0111:B4, como en otras publicaciones del pais. Se destaca el primer aislamiento de Salmonella zaiman en humanos, nueva sero variedad a nivel mundial. Se destaca ademas el primer aislamiento en humanos en la Provincia de Misiones de 1) Campylobacter yeyuni, 2) Cepas enterotoxigenicas de Eschericha coli y 3) Aeromonas hidrophyla. Se apunta la necesidad de continuar con este estudio a fin de colaborar con una mejor vigilancia epidemiologica de las enteritis

Informa del primer aislamiento de Campylobacter jejuni en un nino con diarrea aguda, internado en el Hospital Ramon Madariaga de la ciudad de Posadas, Misiones. Se trata de un paciente de dos meses de edad que en enero de 1987 presento un cuadro de diarrea aguda, con temperatura elevada, deshidratacion moderada, vomitos y 6 a 8 deposiciones diarias. El estudio microbiologico de sus heces resulto negativo para Escherichia coli enterotoxigenica (ECET), eschirichia coli entero invasiva (ECEI), escherichi coli enteropatogena (ECEP) escherichia coli enterhemorragica (ECEH), salmonella, shigella, aeromona y rotavirus, aislandose como unico agente Campylobacter jejuni. Diarrheagenics Escherichia coli are the major agents involved in diarrheal disease in developing countries. The aim of this study was to evaluate the time of appearance of the first asymptomatic infection by the different categories of diarrheagenic E. coli in 44 children since their birth and during the first 20 months of their lives. In all of the children studied, we detected at least one category of diarrheagenic E. coli through the 20 months of the study. 510 diarrheagenic E. coli (33.5 percent) were obtained from the 1,524 samples collected from the 44 children during the time of the study (31.4 percent EAggEC, 28.8 percent EPEC, 27.1 percent DAEC, and 12.7 percent ETEC). Neither EHEC nor EIEC were identified. The median age for diarrheagenic E. coli colonization was 7.5 months. The mean weaning period was 12.8 months and the mean age for introduction of mixed feeding (breast fed supplemented) was 3.8 months. A significantly lower incidence of diarrheal disease and asymptomatic infections was recorded among the exclusively breast-fed rather than in the supplemented and non breast-fed infants. For ETEC, EPEC and EAggEC the introduction of weaning foods and complete termination of breast-feeding were associated with an increase of asymptomatic infections (AU)

A randomized trial of protein supplementation compared with extra fast food on the effects of resistance training to increase metabolism by David Hambre( )

Objective. To prospectively evaluate the effects of resistance training combined with increased energy intake or protein-supplementation on lean body-mass, resting metabolic-rate (RMR) and cardiovascular risk factors. Methods. Twenty-four healthy males (aged 19-32 years) performed resistance exercise for 12 weeks aiming for at least 1 hour training-sessions 3 times a week. The participants were randomized to consume extra protein (33 g whey protein/day) or a meal of fast-food/day (1350 kcal, 41 g protein). Body-composition was measured with Dual-Energy X-ray Absorptiometry (DEXA) and RMR by indirect calorimetry. Fasting blood samples were drawn before and after the 3-month training period and after 12 months. Results. The body weight increased from 75.1 +/- 6.9 kg to 78.7 +/- 7.2 kg (p andlt; 0.0001), without differences between the groups. RMR increased from 1787 +/- 143 kcal/24 h to 1954 +/- 187 kcal/24 h (p andlt; 0.0001, N = 24), which was more than expected from the increase in lean body-mass (increase from 59.7 +/- 4.3 kg to 61.8 +/- 4.1 kg p = 0.004). Fasting serum-insulin levels increased in the fast-food group compared with the extra-protein group (p = 0.03). ApoB increased from 0.691 +/- 0.14 g/L to 0.768 +/- 0.17 g/L, p = 0.004, in the fast-food group only. Long-term follow up after 12 months showed that RMR, body weight, total fat and lean body-masses did not differ from baseline (n = 19). Conclusions. Resistance training for 12 weeks increased RMR and lean body-mass similarly when based on either an increased energy-intake or protein supplement. However, the increase in RMR was higher than expected from the increase in lean body-mass. Thus resistance training could potentially decrease the risk of obesity by induction of increased RMR

Esta Salmonella es una nueva serovariedad aislada del arroyo Zaiman que recorre zonas suburbanas de Posadas (AU)

El presente trabajo informa de la calidad y susceptibilidad de los germenes involucrados en la infeccion nosocomial durante el ano 1988, en pacientes internados en el Sector Pediatria del Hospital Ramon Madariaga. Sobre un total de 1749 muestras clinicas estudiadas, se obtuvo un 23,44%de positividad. El tipo de infeccion mas frecuente fue heridas quirurgicas y tejidos blandos superficiales, como es de esperar en la poblacion pediatrica. Los agentes etiologicos predominantes fueron los bacilos gram ( - ) con marcada resistencia a antibioticos betalactamicos y gentamicina

Dada la aparicion de un brote de infeccion por Serratia marcescens en las Unidades de Ciudados Especializados de un Sanatorio de la ciudad de Posadas; se realizo una investigacion epidemiologica del brote, que permitio evaluar la importancia del mismo, detectar factores de riesgo y algunaos de los probables mecanismos de transmision de la infeccion. Entre el 28 de mayo y el 23 de junio de 1986, ocurrio un brote de infeccion de Serratia marcescens en pacientes internados en los sectores de Unidad Coronaria y Terapia Intensiva. Dada las caracteristicas de las cepas aisladas se analizaron las soluciones antisepticas y parenterales y sus envases (18), arrojando resultados negativos en todos los casos, excepto en un envase que contenia un desinfectante derivado del amonio cuaternario (marca DG 6) donde se aislo la misma cepa con identico antibiotico-tipo. Se implementaron las medidas tendientes a limitar el brote y evitar nuevos casosn cambio en la tecnica de manejo de soluciones antisepticas, extremar cuidados de tecnica aseptica en el manejo de estos pacientes y fundamentalmente cambios en la conducta de quienes manejan estos enfermos. Se destaca como fundamental las medidas de higiene y lavado de manos en la prevencion de la Infeccion Nosocomial

Ninos menores de cinco anos de edad de dos comunidades de diferente condicion socio-economica (97 del Barrio Zaiman y 55 del Barrio Las Dolores), fueron vigilados epidemiologicamente durante 2 anos, mediante visitas trimestrales del equipo de trabajo, el que realizo la toma de muestras fecales. Durante el estudio, se identificaron uno o mas enteropatogenos en el 73,9 por cento de las muestras de ninos del Barrio Zaiman y en el 58,3 por cento de Las Dolores, estando asociados a diarrea en un 70,5 por cento y en infecciones asintomaticas en un 65,7 por cento. El numero de episodios de diarrea fue superior en Zaiman (15,5 por cento) que en Las Dolores (12,4 por cento), presentandose con mayor frecuencia en los meses de primavera-verano ... (AU)
