Cell proliferation and matrix protein accumu!ation overtake normal healing and lead to chronic fibrosis. Chemically modified dextrans, heparan sulfate mimetics, are elaborated by controlled grafting on a native dextran of carboxymethyl and sulfate units (RG1503) and of carboxymethyl, sulfate and benzylamide units (RG1 192).These polymers or RGTA for ReGeneraTing Agents promote in vivo tissue healing, modulate in vitro cell proliferation and regulate collagen biosynthesis. The effects of RGTA were studied in some models offibrosis. A radio-induced fibrosis, and pathologic fibrosis like Crohn's disease and the fibrosis of the vascular wall associated with atherosclerosis. In vitro gamma irradiation of Human Intestinal Smooth Muscle cells with a 60 source, induce the expression of a fibrotic phenotype. Twenty four hours after irradiation, collagen I biosynthesis and pericellular TGFB1 are increased whereas collagen V biosynthesis is decreased only 96 hours after irradiation. Both RGTA regulate III biosynthesis and largely reduce pericellular TGFB1 by irradiated cells, however only RG1 503 acts on co V biosynthesis. In ileum tissue resection from patients with Crolm's disease, there is a marked increase of collagen biosynthesis, particularly collagen III. RG1503 corrects partially collagen III biosynthesis and increases collagen V production. Interactions between RG1503 and TGFB1 have been studied on the collagen expression by vascular smooth muscle cells. TGFB1 increase specifically and synergistically the collagen V stimulation induced by RG 1503. Moreover, TGFB1 antibodies totaily inhibit RG1503 activity on collagen V. These results underline the antifibrotic activity of RG1503, via interaction with TGFB1. RGTA could constitute a new class of potential therapeutic agents in the treatment of fibrotic pathologies
