Introduction: Chronic obstructive pulmonary disease (COPD) has a detrimental effect on the performance of daily activities and consequently on health-relaCed quality of life (HRQOL). Different HRQOL instruments have been developed to quantify the impact of COPD on a patient's daily life, and are used in clinical trials to measure the changes in HRQOL. One of these instruments is the St. George's Respiratory Questionnaire (SGRQ). The basic purpose of this research is to compare HRQOL between patients on home long-term oxygen treatment (h-LTOT) and COPD patients who do not need such treatment yet. The study determines the correlation between the results of theSGRQ, the six-minute walk test and lung function parameters. Methods: All participating patients were examined at the University Clinic of Respiratory and Allergic Diseases Golnik. The examination included a pulmonary function test, a six-minute walk test and the SGRQ. Results: Patients on h-LTOT can walk 159 m less than patients without h-LTOT, and their average forced expiratory volume in 1 second (FEV I) is 800 while FEV 1 of patients without h-LTOT is 1600. The total SGRQ assessment is higher by 28% which means a lowerquality of Iife. The biggest difference in the assessment of SGRQ categories appeared in the subgroup dealing with activities. The comparison ofthe assessment of the quality of life and of pulmonary function shows that the correlation between the two is counter proportionate. The lower the pulmonary function, the lower the assessment of the quality of life. The strongest correlation is that between the distance that patients can walk and the SGRQ assessment: the lesser the distance they can walk, the higher is their % in SGRQ, which means a lower quality of life. % FEV 1 moderately correlates with the distance the patients can walk; the higher it is, the greater the distance patients can walk. Background: Chronic obstructive pulmonary disease (COPD) is a disease that affects not only lungs but also other organ systems. Patients' knowledge about the disease, its treatment and inhalation therapy is incorrect and insufficient which has a negative impact on their quality of life. Self-care is an important concept that can improve patients' quality of life. Methods: We analysed 64 questionnaires filled out by COPD patients in order to assess the effect of health education at the University Clinic of Respiratory and Allergic Diseases Golnik. Literature review was conducted using the international database Medline, Pubmed. Key words for the search were education, COPD, management, quality of life. On the basis of abstracts, further selection of articles and data was carried out. For data analysis, descriptive methods for the distribution of variables, frequency, arithmetic mean, standard deviation and graphical representation in the form of "box and whisker plot", t-statistic for difference testing and Pearson's correlation coefficient. Data was processed using the statistical computer program SPSS V.16.0.1. Results: In both reference years patients were least familiar with the symptoms of the disease and worsening of these symptoms. When comparing the level of knowledge about the symptoms of the disease in the years 2008 and 2009, the difference has proven to be statistically relevant (p<0,05). Discussion: COPD patients have insufficient knowledge about their disease. Continuous health education and patient management are required. Discharge coordinator intervention prevents hospitalizations in patients with COPD a randomized controlled trial( )

Objectives: To test whether coordination of discharge from hospital reduces hospitalizations in patients with chronic obstructive pulmonary disease (COPD). Design: Randomized controlled clinical trial. Setting: Specialized pulmonary hospital. Participants: Patients hospitalized for an acute exacerbation of COPD. Intervention: Care as usual included routine patient education, supervised inhaler use, respiratory physiotherapy, and disease-related communication. The discharge coordinator intervention added assessment of patient situation and homecare needs. Patients and caregivers were actively involved and empowered in the discharge planning process, which was communicated with community medical professionals to provide continuity ofcare at home. Measurements: The primary end-point of the study was the number of patients hospitalized because of worsening COPD. Key secondary end-points were time-to-COPD hospitalization, all-cause mortality, all-cause hospitalization, days alive and out of hospital, and health-related quality of life. Results: Of 253 eligible patients (71 plus minus 9 years, 72% men, 87% GOLD III/IV), 118 were assigned to intervention and 135 to usual care. During a follow-up of 180 days, fewer patients receiving intervention were hospitalizedfor COPD (14% versus 31%, P .002) or for any cause (31% versus 44%, P .033). In time-to-event analysis, intervention was associated with lower rates of COPD hospitalizations (P .001). A Cox model of proportional hazards, adjusted for sex, age, GOLD stage, heart failure, malignant disease, and long-term oxygen treatment, demonstrated that intervention reduced the risk of COPD hospitalization (hazard ratio 0.43, 95% confidence interval 0.24e0.77, P .002). Conclusion: Among patients hospitalized for acute COPD exacerbation, discharge coordinator intervention reduced both COPD hospitalizations and all-cause hospitalizations
