Abstract:

The purpose of this presentation is to discuss and demonstrate the indications and technique for diagnostic proctoscopy and to present some illustrative findings which can be expected to be seen in clinical practice. This objective is achieved first in two step-by-step demonstrations of the procedure on clinical subjects. In this presentation the specific indications for proctoscopy are discussed first. These indications include bleeding from the anal canal, the presence of blood, pus or mucus in the stool, a change in bowel habits, pain, in order to obtain material for smears, cultures or biopsies, and in preparation for colon or pelvic surgery. The only contraindication is the inability to introduce the scope because of an anal stricture. According to this presentation no preparation is necessary. Saline or plain tap water enemas may, however, be given the morning of the examination. In the office the procedure is easily performed using a tilt-down table. An elderly male patient is presented with ulcerative colitis. The digital examination is performed and then the well lubricated proctoscope is inserted through the anal canal in the direction of the umbilicus and the obturator is removed. The scope is then advanced and the examination is done under direct vision. The most important part of this examination is done as the scope is withdrawn; at this time the valve edges are flattened out and the surfaces behind them are examined. The knee-chest and lateral positions are also demonstrated. The normal bowel is shown first, and then proctoscopic views of such conditions as ulcerative colitis, melanosis coli, carcinoid tumor, adenomatous polyp, villous tumor, and carcinoma are presented and discussed.
