St. John's Church was established around 1784 as a German Reformed congregation. It may have become a union church with the joining of a Lutheran congregation. The Lutherans left St. John's in 1839 and started the Mt. Tabor Church. St. John's is known today as St. John's Reformed Church - United Church of Christ.

The church is located two miles from Middlebrook in Augusta County, Virginia.
